This form collects your name, email, optional phone number and message to answer appointment and general enquiries. Submissions are processed by Netlify Forms and may be viewed by authorised site administrators. Do not send sensitive health data, reports, identity details or documents. The WhatsApp button transfers your message to WhatsApp; you send it yourself in the app. Form data is not persistently stored in your browser. Contact the listed phone number to request deletion of your enquiry or data. Google Maps loads automatically when the contact page opens. YouTube players load automatically on dedicated video pages; videos elsewhere open with the play button. These services may process connection and device information under their own privacy policies.
