AI for Embryo Selection: What Can It Really Offer?

When several embryos are available, deciding which to transfer first matters. AI can support this assessment, but its score is not a guarantee of pregnancy.

What does AI assess?

These tools score embryo images and, in some systems, development over time. Their aim is to support transfer ranking. A score does not describe every biological feature or independently replace clinical and embryologist assessment. [1]

The 2024 trial

In a randomised trial of 1,066 patients, the system studied did not meet the prespecified non-inferiority criterion for clinical pregnancy. This does not prove every AI system ineffective; clinical performance must be demonstrated for the particular tool. [1]

A different finding reported in 2025

A seven-centre trial abstract evaluating another system reported meeting its non-inferiority criterion against traditional selection. The study was not designed to establish superiority. Results were reported as a scientific abstract, with methods and analyses also available from the developer. [2] [3]

Interpreting a score

Ranking embryos and establishing their genetic constitution are different questions. Do not treat an image score as a genetic test result. Ask which patients the tool was tested in, whether an embryologist can override it and what it would change in your situation.

Setting expectations

Distinguish clinical pregnancy from live birth. Percentages from different studies cannot simply be combined to calculate your own chances. Ask your team to explain your embryo report, the reason for transfer order and the expected contribution of any additional paid assessment.

Frequently asked questions

Can AI guarantee pregnancy?

No. Supporting embryo assessment does not guarantee pregnancy or live birth.

Does it replace genetic testing?

Image assessment is not a genetic test result. Whether genetic testing is indicated requires a separate clinical assessment.

What if I have only one embryo?

The question of ranking several embryos changes. Ask what the tool would contribute to your particular decision.
